Passport has been lost or stolen, what should I do?

Should a passport be lost, stolen, or destroyed while still valid, the holder is required to submit a police report with such claims. 

When a passport is lost / stolen, Section 7 on Form A must be filled in, signed, and stamped by a recommender even when a passport was expired.

The applicant would need to take an oath or give a sworn declaration to the Passport Office. If the passport was still valid an extra fee of €11 applies.
